How many fixture groups there are. Each bathroom group is another set of points to isolate and another stretch of filling and watching. Two bathrooms in a big house makes for a shorter appointment than four bathrooms in a small one, which is exactly why the estimator asks about bathrooms and ignores floor area.
What the foundation lets the crew see. Under a pier-and-beam house a technician can physically inspect the drain run. Under a slab there is nothing to look at, so the gauge and the camera carry the whole diagnosis on their own. Same test, very different amount of information in hand when the result comes back.
Whether anyone else has to read the result. A test you commission purely for your own reassurance is the cheapest version of this. A test whose output has to stand up in front of a lender, a buyer, an adjuster or a foundation warranty claim includes a formal write-up, and that document is priced as part of the job.
Whether you will need it more than once. Foundation work is the common case. One test records the condition beforehand, another confirms nothing broke while the piers went in. Homeowners are frequently caught out by paying for this twice inside a single year.
WHAT THE APPOINTMENT ACTUALLY INVOLVES
On the drain side, a technician opens a cleanout, drops an inflatable plug in below it to close the line off, and fills the system with water back up into the house. Then everyone waits and watches the level, generally somewhere between a quarter and a half hour. A level that stays put means the line is sound. A level that falls means there is a crack or a break underground somewhere. What the test will not do is show you the location, and that is precisely why a camera run gets added on so often.
The supply side works differently. That system gets isolated and put under pressure, with air or with water, and a gauge is watched for any drop. Running the drain test and the supply test in the same visit is what the trade calls a dual test, and it is the version insurers and leak investigations normally ask for.

What is the estimator based on?
